NEW REQUIREMENT:

All new applicants must provide a 'passport-style' photo, certified by a third party as a true likeness of the applicant, i.e. a clear, current pic, head and shoulders, no hats or sunglasses.

All members renewing their membership must also provide a 'passport-style' photo if we don't have one already.

Membership Application

This explains our rules and your responsibilities as a client of the Dispensary. This is a mandatory form.

Practitioners Statement of Diagnosis
Your health care provider must fill out this form to confirm your diagnosis and/or recommend the use of cannabis as a medicine.

Letter to Doctor
This is an optional letter for your health care provider which explains the Vancouver Dispensary Society's purpose.

Medical Information Release Form
If needed, this form empowers your health care provider to send us your confirmation of diagnosis.

Health Canada Release Form for Medical Practitioners
If needed, this is a form from Health Canada which reduces your doctor's liability in regards to your use of medicinal cannabis.
